PT-141 + Melanotan II Blend
How this combination works
PT-141 (bremelanotide) and Melanotan II are both synthetic α-MSH analogues that act as non-selective melanocortin receptor agonists. This is not a complementary pairing. The two drugs hit the same receptor family, MC4R in particular, so community users combining them are effectively stacking two agonists of the same target. The likely result is overlapping effect and additive side effects, not separate mechanisms. Melanotan II adds strong MC1R pigmentation activity and has an extensive case-report record of serious harm. This rationale is community practice and mechanistic inference only.
Combined mechanism
Bremelanotide is a cyclic α-MSH analogue that acts as a non-selective melanocortin receptor agonist, with MC4R considered the most relevant subtype at therapeutic doses. Animal studies suggest it activates presynaptic MC4Rs in the medial preoptic area, increasing dopamine release. Melanotan II is a cyclic heptapeptide α-MSH analogue that acts as a potent non-selective agonist at MC1R, MC3R and MC4R. At MC1R it stimulates eumelanin production. Through MC4R/MC3R in the central nervous system it induces erections and sexual stimulation and can induce thermogenesis. It also activates the HPA axis. Both compounds converge on MC4R, so their central effects overlap rather than complement each other. No human pharmacokinetic or pharmacodynamic data exist for the combination.

Safety considerations for this combination
- No combination-specific safety data exist for PT-141 + Melanotan II. The signals below come from each compound studied on its own. Component approved files
- Overlapping receptor activity: both compounds are MC4R agonists, so the adverse effects they share — nausea, flushing, blood pressure increase, hyperpigmentation — may be additive. No combination safety data exist. PT-141: PMID: 35147466. Melanotan II: PMID: 11035391, 23121206
- Nausea: most common adverse event (40.0% bremelanotide vs. 1.3% placebo in Phase 3 double-blind integrated studies); most common reason for discontinuation Inherited from PT-141 (PMID: 35147466)
- Flushing: 20.3% bremelanotide vs. 1.3% placebo in Phase 3 double-blind integrated studies Inherited from PT-141 (PMID: 35147466)
- Headache: 11.3% bremelanotide vs. 1.9% placebo in Phase 3 double-blind integrated studies Inherited from PT-141 (PMID: 35147466)
- Injection site reactions: 5.4% bremelanotide vs. 0.5% placebo in Phase 3 double-blind integrated studies Inherited from PT-141 (PMID: 35147466)
- Sympathomimetic toxicity (tachycardia up to 146 bpm, hypertension, diaphoresis, mydriasis, anxiety, diffuse muscle tremors) after subcutaneous self-injection Inherited from Melanotan II (PMID: 23121206)
- Rhabdomyolysis (peak CPK 17,773 IU/L) and acute renal dysfunction (creatinine 2.25 mg/dL) after subcutaneous self-injection of 6 mg Inherited from Melanotan II (PMID: 23121206)

Contraindications
- History of melanoma or multiple dysplastic nevi. Melanotan II has been associated with eruptive and atypical nevi and with melanoma in case reports, and PT-141 causes focal hyperpigmentation with repeated dosing. Melanotan II: PMID: 19380666, 24355990. PT-141: PMID: 35147466
- Uncontrolled hypertension or cardiovascular risk. PT-141 produces transient blood pressure increases, and Melanotan II has caused sympathomimetic toxicity with tachycardia and hypertension. PT-141: PMID: 35147466. Melanotan II: PMID: 23121206
- Caution advised in patients at risk of cardiovascular disease; blood pressure should be well controlled during treatment due to observed transient BP increases Inherited from PT-141 (PMID: 35147466)
- Caution advised due to limited efficacy, potential adverse effects, and transparency issues in reporting; pharmacotherapy should be used within a multidisciplinary approach Inherited from PT-141 (PMID: 38767282)
- Pregnancy and lactation: listed as subject headings in a drug review (Vyleesi for HSDD), indicating special populations of concern; no explicit contraindication text available from the abstract Inherited from PT-141 (PMID: 31381550)
- History of melanoma or multiple dysplastic nevi: synthetic α-MSH peptides can drive proliferation of neoplastic melanocytic cells in predisposed patients, presenting increased risk of melanoma development Inherited from Melanotan II (PMID: 19380666)
- General contraindication for use in any individual given unregulated, unlicensed status; lack of established human safety profile and association with severe adverse events including rhabdomyolysis, renal infarction, and malignant melanoma Inherited from Melanotan II (PMID: 42757290)
- PT-141 + Melanotan II has not been evaluated as a combination in any clinical trial. The lack of combination-specific contraindications reflects that absence of study, not established safety. No combination study cited in either component file
